The Day Center
Designed to be a cost-effective alternative to home health services, nursing home and assisted living facilities, Evergreen's state-of-the-art adult day health center offers a daytime program of nursing care, social services, nutritious meals and therapeutic activities for adults requiring assistance.
The specially trained staff at The Day Center of Evergreen Commons provide a safe, caring environment for our participants and relief for their caregivers during the day, enabling individuals to live at home and remain independent in their communities. We are dedicated to maintaining – and often improving – the functional levels of our participants to help make life more enjoyable for everyone involved.

Each individual participant has a therapeutic activity plan of care based on his or her Interest Survey, Health History, and Health Assessment. The activity plan will emphasize the participant's strengths and abilities and contribute to feelings of competence and accomplishments. Activities include, but are not limited to:
Creative Arts: painting, drawing, crafts
Educational: brain builders, trivia, computer games
Music: bell choir, musical performance guests, sing-a-long
Physical/Gross Motor: armchair exercise, balloon swat, bowling
Social: front porch travels, discussion group, reminiscing, table games
Spiritual: faith and devotions, prayer circle, hymn sing (we also offer these groups in Spanish, when possible)

The Day Center of Evergreen Commons charges each participant according to their level of care. An assessment is completed upon admission to determine what level of care a participant may be, ranging from Level 1 (least amount of additional care) to Level 3 (highest level of additional care).
The private pay costs are either for a full day (more than 4 hours) or a half day (4 hours or less).
• Level 1: $90/Full Day (equivalent to $11.25/hour), $65/Half Day (equivalent to $16.25/hour)
• Level 2: $100/Full Day (equivalent to $12.50/hour), $70/Half Day (equivalent to $17.50/hour)
• Level 3: $110/Full Day (equivalent to $13.75/hour), $75/Half Day (equivalent to $18.75/hour),
***Funding assistance may be available for those who qualify through the following:

IS ADULT DAY CARE COVERED BY LONG-TERM CARE INSURANCE?
Most long-term care insurance policies do cover adult day care services. Check your policy to be certain. Often there is a co-pay or required amount of money that must be spent out of pocket before the long-term care insurance begins paying.

WHO TAKES CARE OF MY LOVED ONE WHILE THEY ARE AT THE DAY CENTER OF EVERGREEN COMMONS?
Rest assured your loved one will receive incredible care from our experienced staff members. Our team consists of a Director, Program Nurse, Activity Coordinator, Caregiver Support Coordinator, Health Support Coordinator, Food Service Coordinator, Floor Managers, Medication Technicians, and Program Assistants. Each Day Center staff receives training specific to adult day services and are required to maintain CPR and AED certification. Background checks are completed on all staff members before employment in our program begins.
WHAT IS YOUR STAFF-TO-PARTICIPANT RATIO?
The Day Center of Evergreen has at least one staff member for every five participants.
ARE MEALS PROVIDED AT THE CENTER?
Yes! We partner with AgeWell Services, who provide healthy lunches each day.
DO YOU OFFER TRANSPORTATION SERVICES?
The Day Center of Evergreen Commons offers transportation services. We have partnered with Pioneer Resources and Allegan County Transportation to be able to provide busing services for our participants. Our goal is to provide you with safe, personable, and high quality service from your front door to ours.
