Case Coordination

Our trained, professional social workers provide in-home assessments, coordinate necessary services, and provide on-going monitoring for frail, at-risk seniors to ensure their well-being and safety.

Telephone Reassurance

Trained volunteers will make scheduled phone calls to the homebound to provide friendly conversation and to check on their safety.

Ottawa County Congregate Meal Program

This program offers lower income seniors, 60 or older, the daily Evergreen meal served in our dining room, with payment in the form of a meal voucher.
